Is a ceramic blade sharp enough for hair designs?

Ceramic is harder than steel and takes a very fine edge, which is why it holds that edge much longer. For design work the thing you actually want is an edge that does not degrade over the course of the design, and that is exactly what ceramic gives you.

How careful do I have to be with it?

Handle it like a good blade. Ceramic will not rust and will not corrode, but it does not like being dropped on a hard floor. Everyday station use is not a problem.

Does it change how I set the blade?

The procedure is identical, but be precise. Ceramic will not wear into a slightly wrong setting the way a soft steel blade might, so get the level right before you use it.
