How often should the jar be changed?

Change the solution daily, or immediately whenever it becomes visibly cloudy or contaminated. Regulations vary by state, so follow your board's rule if it is stricter.

Can I use it on surfaces as well as tools?

The label covers immersion of non-porous tools and surface use where the surface is left visibly wet for the full contact time. For fast surface work most shops use Barbicide wipes instead.
